PROGRAMS WE’LL PROVIDE

A smiling elderly couple with the woman in a wheelchair, both laughing, against a peach background with the word 'PASSPORT' written at the top.

PASSPORT Program

  • The Ohio PASSPORT Medicaid program is a long-term care option designed to help elderly individuals maintain independence and receive necessary services in their own homes rather than in a nursing facility.

PASSPORT offers a range of services such as personal care, homemaking, transportation, and nursing care, tailored to meet the specific needs of each participant. This program aims to enhance the quality of life for seniors by enabling them to remain in familiar surroundings while receiving the support they require, promoting autonomy and dignity in their later years.

  • Call 812-247-6017 for PASSPORT Program rates. Rates depend on level of care required, but we pay the highest PASSPORT rates in the state for family caregivers.

Ohio Individual Options Program

Individual Options (I/O) Program

  • The Ohio Individual Options (I/O) Medicaid program provides individuals with developmental disabilities the opportunity to receive home and community-based services, allowing them to live independently or with family rather than in institutional settings.

This program offers a variety of support services tailored to the unique needs of each participant, including personal care, respite care, transportation, and specialized medical equipment. By emphasizing individual choice and self-direction, the I/O program aims to empower individuals with disabilities to lead fulfilling lives within their communities, promoting inclusion and quality of life.

The program is managed by the Ohio Department of Aging and is part of Ohio’s effort to help seniors live independently.

  • To qualify for the Medicaid waiver program Ohio, individuals must be 60 or older, meet certain income and asset limits, and require a level of care typically given in a nursing home. • Yes. If your loved one qualifies for PASSPORT and selects a self-directed care option, they may choose a family caregiver to provide services at home. This helps delay or prevent the need for a nursing home stay, which can be more costly and less personal. Support includes help with activities of daily living, such as toileting, grooming, and mobility.

  • The Ohio Department of Aging oversees the PASSPORT program and partners with local Area Agencies on Aging to deliver services. They monitor eligibility, provider networks, and ensure that the Medicaid waiver guidelines are followed to support seniors safely in their homes.
