Community Care Program

  • The Illinois Community Care Program (CCP), funded by Medicaid, helps seniors and disabled individuals stay home by offering financial assistance for in-home care. This can be a great option for families!

    Family members can become paid caregivers through CCP, receiving reimbursement for essential tasks like bathing, dressing, and medication management. CCP can also provide caregiver training and offer peace of mind through financial support.

    All CCP caregivers are trained and paid through home care agencies like Paid.care.

Family HHA/CNA Illinois Program

  • Home Health Aide (HHA) or Certified Nursing Assistant (CNA) programs enable caregivers to undergo training and certification, qualifying them to provide professional care to their loved ones.

    Through Medicaid, caregivers can receive compensation for their services, ensuring they are adequately trained and financially supported while caring for family members in need.

We work with Illinois Department of Healthcare and Family Services (HFS) and families to assist elderly and disabled individuals in obtaining Medicaid or Medicaid Waivers. You can receive payment through Paid Care and one of the several Illinois Medicaid Waivers.

Who Is Eligible?

1. Live in Illinois

You must live in the State of Illinois and live in Cook County, Will County, or Kane County.

2. Live Near or With Your Caregiver

For the Community Care Program and Persons with Disabilities Programs, your caregiver has to live in the same house or apartment as you.

3. Additional Medicaid Requirements

Age 60 or older for the Community Care Program, Medicaid eligible, Be at risk of nursing facility placement as measured by the Determination of Need (DON) assessment, and can be safely maintained in the home or community-based setting with the services provided in the plan of care.
